Q: I am looking for a USCIS Civil Surgeon.  Is Dr. Patel a certified designated USCIS Civil Surgeon?

  • Yes, Dr. Dhiraj Patel is a certified designated USCIS civil surgeon. 

Q: How long does it take for me to receive my sealed I-693 form?

  • It takes about 5-7 business days, unless any of your lab tests come back abnormal. If additional tests need to be done, it may take 8-10 business days.

Q: What is the cost of having my I-693 examination in your office?

  •  $450.

Q: What is included in the fee for the immigration physical exam?

  • Immigration physical examination and completion of your I-693 form.

  • Blood testing: Quantiferon (Tb test), Syphilis testing, Urine testing, MMR and varicella titer testing.

  • Flu vaccine and TDAP vaccine included

Q: What is NOT included in the $450 cost?

  • Vaccines NOT included: MMR and Varicella if needed . These vaccines would have to be completed at an outside pharmacy or health department (your insurance may cover this - please inquire with them).

  • Chest X-ray is NOT included. It will not be required if your TB test negative. If your TB blood test is positive, this would be done at an outside radiology facility, and would incur extra cost.

Q: Is there a discount if I have all of the vaccines or if I've had a prior I-693 which expired?

  • No. The cost is the same.

Q: Do you accept medical insurance or travel insurance for the immigration physical exam?

  • No. The I-693 exam is NOT covered by health insurance. 

Q: How can I prepare for the immigration physical exam at your office?

  • Call our office to make an appointment. Do not make an online appointment from our website. Immigration physicals require prolonged appointment slots.

  • Complete our secure immigration registration form prior to your appointment.

  • Bring your government issued photo ID (drivers license or passport) and any USCIS ID you've been issued (green card or work permit) to your appointment.

  • Bring vaccination records and any pertinent medical records if you are being treated for a disease or disability to your appointment

Q: Can I undergo the exam if I do not have my vaccination records?

  • Yes. If you do not have any, you may get all of the required shots if the blood work we perform does not reflect immunity.
